Ninja Gaiden (Xbox) (I just got this over the weekend, and I'm in chapter 5 i believe)
Metroid Prime (Cube)
Metal Gear Solid: The Twin Snakes (Cube)
The Legend of Zelda: Wind Waker (Cube)

Thats a hard one, I made it to chapter 4 before I took it back to Hollywood Video. Those damn green ninja's kicked my ass so much.

Mostly oldies. I deal with mostly imports, so I would say, "Radiant Silvergun" for the Sega Saturn.

And I guarantee that most old time gamers remember how hard it was getting past the third stage in "Ghouls and Ghosts." I played it for the NEC SuperGrafx
Modern, I would agree with most, and say Ninja Gaiden for X-Box. There have been many a injury sustained from playing that game.
